EnrollmentWe would love to have your child(ren) join us at one of our centers and thank you for entrusting the care of you child(ren) to us. Our staff is committed to nurturing the growth and development of children while keeping them safe. We strive to foster their natural curiosity and encourage their independence as they explore their environment and build upon their kills to become lifelong learners and community members.
If you would like to schedule a tour or seek additional information on our centers, rates, or programs, please call one of our locations or fill out the contact request form. 
The following are required to be on file by the Wisconsin Department of Children and Families while your child is enrolled at our center. On or before your child's first day of attendance, please fill out and complete the following forms:
The Child Health Report and Immunization record can be turned in up to 90 days after the first day of attendance. Please make sure a physician signs the Child Health Report.
If your child will be riding the bus to or from our center through Johnson School Bus Service, please fill out the Transportation Permission form below.
Stepping Stones also requires additional forms to be completed upon the child's enrollment. These forms can be obtained from one of our locations and/or signed on your child's first day of attendance.
